Once printed, fill-out and sign both documents and fax back to (312) 896-3795. Documents can also be scanned and emailed back to info@bellainvestmentgroup.com. PLEASE NOTE: Bella Investment Group, LLC collects a fully refundable $500 due diligence deposit at the time of formal application submittal. This amount is mandatory and must be received before any further work can be preformed on a loan application. In the even that we are unable to provide financing and all other conditions have been met, this fee will be refunded in accordance with the stipulations outlined in the Disclosure Statement. Once the above documents have been received, we will need to receive the refundable due diligence and document preparation fee as outlined in the Disclosure Statement.
